Is this Tochinohana or Tamanoshima? As you got the picture obviously from a Japanese newspaper (sponichi) it would have helped if you'd include a link to the article, so that I (or other) could read it and maybe give you the answer...

Tochinohana suffered an intense back muscle spasm which goes with the name lumbago too. He has history of lower back problems. It was unprecedented that during shitaku-beya procedures rikishi gets so bad cramp that has to withdraw from basho. Evidently he was just doing normal stuff, not even shiko or anything, and then got this violent back muscle spasm which caused a lot of pain, inability to move and face grimacing. Kasugano-oyakata says re-participation is possible but not likely.

Is there a problem with Tamanoshima aswell then Doitsu?

I can't answer for Doitsu, but Tamanoshima looked in a lot of pain immediately after his bought with Kaio yesterday. A lot of pain. He stayed down longer than usual and was holding his left elbow/arm afterward. I was surprised to see him still on for today.

Kaio put his patented Krunch on Tamanoshima, but Shima got down in a hurry and Kaio let up so there was no elbow dislocation as has happened with the Krunch in the past. I too was surprised to see Shima seemingly suffering no ill effects.
